It could even be a worn out bezel or something else. There are even times when the material is there, but it may have come loose due to wear–a good bang will knock a stone out of its setting causing it to fall out.

This bracelet (handmade by a customers grandfather) was refurbished as best as we could, bringing it back to life. The tiny turquoise slivers were hand cut to replace many of the stones and finished off with a traditional oxidation and high polish.
To restore this class ring to its original beauty, this heavily worn gemstone and bezel setting were replaced.
